You receive a registration envelope with a Personal Health Record. Also included are Living Will, Healthcare Surrogate, and HIPPA Release forms. If you need help filling out your PHR, talk with your local eMed-ID representative to assist you. Remember you do not need a computer to complete this process. Once your package is completed you drop it in the mail with the postage paid envelope.
